IMPORTANT CHANGES DURING THE COVID-19 PANDEMIC

The Health & Counseling Center is currently closed to the public now and indefinitely.  We are, regretfully, not able to offer any services. This includes physical exams, immunizations, etc.

If you are an Allied Health student or if you are applying to live on campus or play sports, please have your healthcare provider/former high school fax over your immunization and physical exam records.  They do NOT have to fill out our regular forms during this pandemic.

Clinical Clearance

You start by submitting this Allied Health-specific Health History Form.  Note that the physical exam and immunization pages need to be completed by your healthcare provider.  All required immunizations are listed on that form.

We will also need to have a copy of your insurance card, a copy of your CPR certification and, for those in the Paramedic Program, color blindness testing.

Once all requirements are fulfilled, we will issue your Clinical Clearance Form to be picked up at the Health & Counseling Center.

You will need to get titers done to check for immunity:

  • Hepatitis B Titer
  • MMR Titer
  • Varicella Titer

In addition, you will also need to have the following done: 

  • 2 TB Skin Tests
  • TDAP

You will be required to show your Clinical Clearance Form upon starting your Clinical. Please refer to your Department for any questions about this.

As long as you are compliant in the process of getting them done and as long as you make sure we have a record of each shot as you get them, you will be cleared for clinical. (Health & Counseling fax: 603.230.9308, email: nhtihealthservices@ccsnh.edu)

PLEASE MAKE SURE TO GET A HEPATITIS B TITER AT THE END OF THE SERIES. This is a clinical clearance requirement.

It is ultimately the responsibility of the student to know when their Clinical Clearance expires and what is due. We will update the Clinical Clearance Form within 2 business days of any newly submitted medical information.

CPR

We offer CPR classes on campus for a fee.  Campus CPR classes are compliant for all Allied Health programs.

You are required to take BLS for Healthcare Provider or Professional Rescuer CPR from the American Red Cross, American Heart Association or the National Safety Council ONLY. CPR is renewed every 2 years and must remain current during your enrollment. Online-only classes are NOT acceptable.

Doctor

  • You can get a free physical exam here at the Health & Counseling Center; no insurance required. Please note, however, that this is only available on certain days each semester, September – May.
  • We require scheduling an appointment for a physical exam.  Call us at 603.230.4043 or email us at nhtihealthservices@ccsnh.edu.
  • You can go to any Convenient MD.
  • or CVS Minute Clinic,
  • or Concentra
  • If you do not have insurance, you can come in to the Health & Counseling Center and get a voucher for a free physical exam at any Convenient MD location in the state.

YES. Your doctor (or healthcare provider) needs to fill out the physical exam and immunization portion of the Health History Form.

Insurance

NHTI does not offer insurance.  Students who require insurance can check with private insurance options or apply to Healthcare.gov, which offers affordable insurance options: https://www.healthcare.gov/.

You can get help navigating the Healthcare.gov website for FREE through:

Health Insurance: Proof of health insurance is required by submitting a copy of your insurance card. Active military can submit a copy of their military ID. To be accepted health insurance must meet the following criteria:

  • United States-based insurance plan;
  • Provides the 10 Essential Health Benefitsspecified in the Affordable Care Act;
  • Includes access to hospital and physician providers in the area where the student is attending a New Hampshire community college;
  • Will remain in effect for the entire semester (except for termination due to the attainment of a maximum age, or other situation resulting in a loss of plan eligibility).

The follow plans DO NOT meet the criteria:

  • An accident-only policy;
  • A short-term limited-duration health plan that does not meet the requirements of the Affordable Care Act (ACA);
  • A ministry sharing plan, even if it is recognized by the ACA;
  • Any other health benefits program (e.g., a community care program) that is not recognized by the State of NH as being health insurance (or is not a health benefits plan governed by the Employee Retirement Income Security Act of 1974) and does not meet all of the requirements specified above.

Please be aware that we will need 2 business days to process any new medical information submitted.

YES. In terms of your personal health information, we require you to fill this out: Allied Health-specific Health History Form.  Your doctor or healthcare provider will need to complete the pages for the physical exam and immunizations.

Residence Life and Sports FAQs

Clearance

You submit this Health History Form, and you submit documentation of the following to the Health & Counseling Center: 

  • physical exam within the past year;
  • measles/mumps/rubella (MMR1 and MMR2) after 1980 OR a positive antibody titer (a blood test that shows you have immunity)
  • and a TD or TDAP (tetanus/diphtheria/pertussis vaccine) done within the past 10 years.

You submit this Health History Form, and you submit documentation of the following to the Health & Counseling Center:

HOWEVER, if you are unable to have your healthcare provider fill these out, please send us a record of your physical exam and your immunization record.  You can get this from your doctor or your high school.
