• Textbook required: Ann Brunick and Morris Clark, Handbook of Nitrous
                              Oxide and Oxygen Sedation, Third Edition
• Current RDH license
• Proof of liability insurance
• Current CPR certification
• Medical Health History Form (Download and Print)
